AB AIM: To review the diagnostic yield from the biopsy of macroscopically
normal mucosa seen at upper GIT endoscopy in AIDS patients with GIT
symptoms. METHOD: The case records of 62 AIDS patients having upper GIT
endoscopies between 6/91 and 6/93 were retrospectively reviewed.
Clinical history; endoscopic appearance; histology and microbiology
results were analysed. RESULTS: 147 biopsy results were evaluable; 81
biopsies were taken from mucosa with normal endoscopic appearance.
Significantly abnormal results were obtained from 4 biopsies; 2 of 11
and 2 of 31 endoscopically normal oesophageal and gastric biopsies
respectively. All 4 biopsies revealed cytomegalovirus (CMV) disease. Two
biopsies were from patients with previously diagnosed CMV disease. None
of the 39 macroscopically normal duodenal biopsies revealed
significantly abnormal results. This compares with the diagnostic yield
of 27 of 66 biopsies with abnormal endoscopic appearance. CONCLUSION: In
AIDS patients with GIT symptoms the diagnostic yield from the biopsy of
macroscopically normal mucosa is low particularly in patients without
previously diagnosed GIT disease.
